Did you enter a Dollar ($) Sign? If you are entering a total amount, please remove the dollar sign. Please refer back to your receipt and enter ONLY the dollar amount, followed by a period, followed by the cents. Example: 12.99. **This field may not be applicable to the certificate page you see.

Did you enter a valid Date? The date boxes will not accept any date in the future. Please verify that you are selecting either today’s date, or a date from the recent past. Please refer back to your receipt for the date you visited, and select the Month, Day, and Year from the drop-down menu. **This field may not be applicable to the certificate page you see.

Did you enter a valid Time? Please verify that you have entered the hour of visit in the first box, and the minute of visit in the second box, or have selected them from the drop-down menu (if applicable). Please do not enter any letters, special characters (colons, commas, periods, #, etc), or spaces. Please refer back to your receipt and either enter or select ONLY numeric values.

Please remember to select the AM or PM option (if applicable) beside the Time of Visit boxes. If your receipt is printed in a 24-hour clock (ex. 13:29), please select PM for any time after and including 13:00.

Did you enter your personal telephone number? Please enter the store telephone number located on your receipt, with no spaces, dashes, or letters. Example: 9991234567. **This field may not be applicable to the certificate page you see.

I did everything you said and I still can’t enter! Once all the boxes are filled in, please do not press the “Enter” key on your keyboard, as it will not allow you to gain entry into the survey. You must click the “Enter” button on the survey entry page.
